RELEASE OF CLAIMS: In consideration of the acceptance of my participation in the FHNB July event, I release FHNB, the City of Madison,
county of Dane, Governor Nelson State Park, all respective agents and employees, of the afore mentioned and all others connected with this
event, from and liability or claims for an injury to body or property or illness that I sustain during my participation in this event. I
understand that this release applies to myself, heirs and assigns. I represent that I am capable of participating in this event, recognizing that
risk of injury may accompany such participation and acknowledge that this release is being relied upon by the above persons in permitting
me to participate. I grant full permission to any and all of the foregoing to use any photographs, movies, recordings and other records of this
event, without compensation.
The FHNB event will adhere to all applicable federal, state, county and city guidelines regarding COVID-19. Applicants and participants agree
that safety is the responsibility of all attendees and affirm, by signing this application, that they have vaccinations or oral medications to
prevent the spread of COVID-19. As this event takes place in public place, applicants and participants may be exposed to COVID-19 through
non-attendees. Completion of this application by applicants and participants releases FHNB from any liability for exposure to COVID-19.
